What Are NHS Braces, and Who Qualifies?

Free orthodontic treatment provided by the NHS is only available for those under 18 years old, and they must have a clear need for the treatment. The referral process is structured to ensure that treatment is provided to those who need it most, rather than operating on a first-come, first-served basis.

  • The Index of Orthodontic Treatment Need (IOTN) grades the severity of a child’s bite issues on a scale of 1 to 5. Free braces are offered to grade 4 and 5 patients and are offered on a case-by-case basis for grade 3. 
  • The Aesthetic Component is also considered, which scores the appearance of the teeth on a scale from 1 to 10, with 10 representing the most severe cases.

Typically, the NHS only provides braces for those with moderate to severe orthodontic needs, leaving children with milder issues ineligible; statistics show that roughly ⅓ of children qualify. Additionally, the NHS generally only offers traditional metal braces, as these are cost-effective, though they may not appeal to children seeking less visible options.

How Long Is the Waiting List for NHS Braces?

The current length of the NHS waiting list for braces largely depends on where in the UK you live - some areas are facing much more of a demand than others. In areas facing less of a demand, the waiting list is around 6 months. However, in many cases, the wait could be as long as 1 to 2 years; sometimes even longer!

The COVID-19 pandemic worsened the situation, as it did for many aspects of free healthcare. During the lockdowns, many orthodontic clinics were forced to close or operate at reduced capacity, pushing back appointments and creating a significant backlog of cases - the recovery from this is still ongoing, even today!

One of the most significant challenges created by long waiting times is the risk of children “ageing out” of eligibility for free treatment. For example, if a child is placed on the waiting list when they are 16, they may not receive braces before turning 18, at which point they no longer qualify for NHS-funded orthodontic care. Worsening misalignment issues can cause speech and chewing difficulties, uneven tooth wear, and an increased risk of tooth decay and gum disease.
